It could be said I get a little fastidious when I am sleep deprived, sitting at my desk for hours on end with an unlit-hand-rolled cigarette in my mouth I cut up literally with small medium and large scissors an unfinished essay into smaller and smaller pieces because my red pen went dry and the other one fell behind the dresser.

This little prose sounds too logical for me to believe the person is being sleep deprived. I suggest inserting the phrase, "it could be said" illogically and intermittently through out the piece. Get rid of all punctuation and make some of the sentences into fragment or incomplete thoughts. I need to feel the struggle of an unslept bed.
